Citalopram Warnings & Precautions

Before taking Citalopram, inform your doctor of all your medical conditions and medications. Certain conditions and patient groups require special precautions or may make Citalopram unsuitable.

Key Warnings

  • Monitor for increased suicidal ideation especially in the first weeks of treatment in patients under 25
  • Do not start or stop SSRIs without medical supervision
  • Discontinuation syndrome — must be tapered gradually
  • Risk of serotonin syndrome with multiple serotonergic agents

Pregnancy and Breastfeeding

  • SSRIs are among the most studied antidepressants in pregnancy. Benefits often outweigh risks for moderate to severe depression. Paroxetine has a marginally higher cardiac defect risk and is usually avoided. Neonatal adaptation syndrome may occur. Discuss risks and benefits with your doctor.

Older Adults

  • Increased risk of hyponatraemia, falls (dizziness), and QT prolongation (especially citalopram). Start at lower doses. Monitor sodium levels in first weeks of treatment.

Chronic Conditions

  • Use with caution in bleeding disorders, epilepsy (lowers seizure threshold slightly), or cardiac conduction abnormalities (particularly citalopram/escitalopram and QT interval).

When Not to Use

  • Concurrent MAOI use (14-day washout required)
  • Citalopram/escitalopram: concurrent QT-prolonging drugs or long QT syndrome

Safety Note

Citalopram should only be taken as prescribed. If you have any concerns about whether Citalopram is suitable for you, consult your doctor or pharmacist.
