Chest Pain and Sweating: Causes, Conditions & When to See a Doctor

Chest pain with profuse sweating (diaphoresis) is a hallmark of acute MI. The sweating results from massive sympathetic activation during myocardial injury. Combined with chest pain, this has a positive predictive value > 80% for ACS. Do not dismiss as anxiety or heat in any patient over 35.

Possible Causes of Chest Pain and Sweating

Conditions that commonly cause both symptoms together

  1. 1STEMI or NSTEMI — sympathetic activation causes diaphoresis
  2. 2Unstable angina with intense sympathetic tone
  3. 3Aortic dissection with catecholamine surge
  4. 4Severe hypertensive crisis with sympathetic overdrive
  5. 5Anxiety/panic attack (diagnosis of exclusion after cardiac causes ruled out)

Emergency Red Flags

Seek immediate medical attention if you experience any of these

Cold clammy sweating with pressure-like chest pain
Diaphoresis out of proportion to ambient temperature
Onset at rest or awakening from sleep
Patient describes sweating starting when chest pain began
Sweating + chest pain in diabetic patients (silent MI presentation)

Schedule a medical consultation if you notice these signs

Call 112/911 — do not wait to see if symptoms improve
Aspirin 300 mg chewed if not contraindicated
Loosen tight clothing; rest in semi-recumbent position
Inform emergency team this is a possible ACS for priority response
