Anxiety combined with wheezing is the core presentation of anaphylaxis — the bronchospasm produces wheeze while the systemic allergic response causes intense anxiety and impending doom. This combination also occurs in acute asthma attacks with panic, pulmonary embolism, and vocal cord dysfunction. Anaphylaxis must be the first diagnosis excluded.
